IVF Grant

The Modern Miracle Foundation has partnered with Pollin Fertility in Toronto, expanding our ability to provide financial assistance to Ontario residents requiring IVF treatment to build their families.

Together, we have created the Pollin x Modern Miracle IVF Grant Fund, which awards an $8,000 IVF Grant to Pollin patients who qualify. For every grant awarded, Pollin will also contribute $2,000 in additional funds, bringing the total grant amount to $10,000.

The Pollin x Modern Miracle IVF Grant is available to all who are eligible regardless of the applicant’s sex, gender, sexual orientation, religious beliefs, family or relationship status.

Grant Details

  • The $10,000 grant is a gift. Repayment is not necessary.

  • Applicants are selected based on a number of criteria (eg. income level, medical prognosis). Applicants are not guaranteed a grant.

  • There is a $50 application fee (treated as a donation).

  • Grants are paid and applied to the patient’s invoice at Pollin Fertility. Grants are not provided to recipient directly.

  • The grant must be used within 6 months of application approval date.

  • The grant will not apply to any funded cycles (i.e OFP funded cycle).

  • These are one-time grants used to offset the cost of one IVF treatment cycle.

Appliction Process

  • 1. Review Eligibility Criteria

    Before applying, please ensure you meet the eligibility criteria listed below.

  • 2. Complete the online application

    You will need to complete 3 documents in addition to the online application to be considered for a grant. Find these all linked below in steps 2 & 3.

    We also require a $50 donation that is treated as your application fee.

  • 3. MMF reviews your application

    It may take up to 8 weeks for your application to be reviewed. You will then be contacted via email with respect to the outcome of your application.

  1. Eligibility Criteria

 

Before applying, please ensure you meet all of the following requirements:

1.    Both the applicant and co-applicant (if applicable) must be a Canadian citizen or permanent resident as well as a current resident of Ontario with a valid OHIP card

2.    You must already be a Pollin patient and have completed a consultation and fertility assessment

3.    Annual gross household income of < $150,000 CAD

Note: Grants only apply to IVF treatment and must be used within 6 months of application approval date. The grant will not apply to any OFP funded cycles.

2. Required Documents

 

Please download and complete these documents. You will upload them within your application. We recommend completing these first before you begin the application.

1. Assets & Liabilities Form: Once you complete this form, you’ll input the information as part of your online application and upload a copy of the completed form. Download here.

2. Medical Evaluation Form: To be completed by your treating Fertility Specialist. Once they have completed the form, you’ll input the information as part of your online application and upload a copy of the completed form. Download here.

3. Online Application

 

Complete the online application. While we aim to process applications as quickly as possible, it may take up to 8 weeks to complete the review. You will then be contacted via email with respect to the outcome of your application. Incomplete applications will not be considered.

Submit your $50 application fee which is done via $50 donation on our website. Please put in the notes that this donation is related to your application.

FAQ

  • Once we receive your application, members of the Modern Miracle team will review the information provided. Once the information is verified, we send the signed Medical Evaluation Form to our medical advisors for review.

    Any non-medical information that is included in the Medical Evaluation Form (i.e. applicant name, name of clinic, name of fertility specialist) is redacted and our medical advisors will never review and opine on their own patients.

    Once your application has completed our review process, you will be contacted via email as to the outcome of your application.

  • The grant can be used to offset the cost of one IVF treatment cycle, excluding the cost of medications.

  • While we aim to review applications as quickly as possible, it may take up to 8 weeks to complete the review.

  • Pollin and The Modern Miracle Foundation have a shared commitment to hold annual fundraising events to raise funding for grants as well as awareness about the importance of all Canadians having equitable access to fertility treatments.

    The Pollin x Modern Miracle IVF Grant Fund is separate from Modern Miracle’s General Fund. All funds raised in the Pollin x Modern Miracle IVF Grant Fund go towards grants for Pollin patients only.

    Funds raised by Modern Miracle directly go to Modern Miracle's General Fund and are used to provide grants to patients at any other clinic in Canada.

  • Expenses not covered by the grant include: medications, storage fees, advanced testing fees.

  • Yes, grants must be used within 6 months of your application approval date.

  • No, grants cannot be used to offset any IVF expenses during a government funded cycle.

  • This may be a factor when determining which applicants are selected. Given that we have limited grants to provide, preference may be given to those who do not have access or the opportunity to take advantage of a funded cycle.

  • There are many considerations that go into the applicant selection process. For example, we review financial need as well as the prognosis provided by your doctor. All of these considerations help us with the difficult decision of who is selected for a grant.

  • You can re-apply but we suggest only re-applying if there has been a material change in your application. For example, if your application is rejected due to medical prognosis, we recommend applying if there has been a material change to your treatment plan (eg. from using own egg/sperm to using donor).

  • These grants are one-time grants. As such, you cannot re-apply after successfully receiving a grant from Modern Miracle.
